HeartWise allows continuous communication between patients and their clinical teams, helping the patient feel accompanied whilst providing essential and time-sensitive information to their physician.
This tracking system has three main components that communicates between them: the patient's smartwatch app, the patient's mobile app, and a clinical portal. The app is available on Android and iOS app stores and is designed to be used with Samsung smartwatches.
The case manager monitors the heart rate, symptoms, and patient compliance with the daily tasks and medication intake in the clinical portal, allowing them to keep track of the patient's progress all in one platform.
Patients feel confident they are doing the right thing under the watchful eye of their doctor from the comfort of their own home.
